Forms collect your name, contact email, optional phone number, institution or organisation, and the details you enter for your chosen participation route. Your submission is associated with your signed-in account.

How it is used

The HCC — Healthcare Creators Collective organising team uses these details to review nominations and proposals, plan participation, and handle partnership enquiries. Submitting does not opt you into unrelated marketing. Information is stored in the website’s hosted database and is not shown publicly.

Keep sensitive information out

Do not submit patient identifiers, confidential clinical records, financial credentials, unpublished trade secrets, or private third-party documents. Share only links and information you have permission to provide. All research authors must agree to submission.

Access and withdrawal

Use My applications from the same signed-in account to see your submissions. Withdrawing removes the submitted contact and proposal details from the active database. Your account link, application type, reference, timestamps, and withdrawn status remain as a minimal history. Hosting backups may retain prior versions under the platform’s backup lifecycle.

Confirmation and programme status

Confirmation is displayed on this website; automatic email notifications are not configured. Early applications are received for consideration. The event date is 6 December 2026 at Radisson Blu Hotel, New Delhi Paschim Vihar. Event timings, final eligibility, review criteria, and programme places remain pending. Introductory fees are ₹499 for award nomination (original ₹2,999), ₹1,999 for research participation (original ₹3,999), and ₹499 for student attendance. The research pass includes full summit access, lunch, sessions, and networking. Submitting a form alone does not issue an invitation or ticket. Successful payment guarantees an invitation to attend. It does not guarantee an award, research presentation slot, investment, stall booking, publication, or academic credit.

Payments and partnerships

No payment is taken through these forms. Partnership budgets are indicative only and do not create a booking or financial commitment. Sponsorship is independent of awards and research review.
